Medicaid, Medicare, Independent Health, Community Blue, Blue Cross/Blue Shield, Choice Blue, Community Care, Champus, Choice Care, GHI, Univera, Empire and Labor Health

A sliding fee based on income is available for uninsured individuals and their families residing in the towns of Evans, Brant, Collins, North Collins & Eden.

Behavioral Health Clinic services are subsidized by New York State, the Towns of Evans & Brant, individual and corporate donations.  

Services are provided in accordance with laws governing personal rights and confidentiality.
